Case
Coordination
Most
people want to live in their own home as long as possible.
Case Coordinators work individually and confidentially with
people to determine what services they need to remain living
independently. They work one-on-one with a client unless
the client wishes to involve spouses and/or other family
members. Each client is unique in what services they need
and Case Coordinators work with other area agencies to secure
those services.
Referrals come from a variety of sources. Sometimes the
person calls looking for services. Sometimes children, both
local and living long distance, call concerned about their
parent. Churches, friends, and other agencies make up the
balance of referrals.
Case Coordinators can determine in the initial phone call
whether or not the person qualifies for any Federal or State
funded programs. If the person’s income exceeds these
programs, then Case Coordinators can work with the client
on a fee base. Complete assessments are conducted in the
client’s home, not over the phone.
To learn more about this program or to make a referral,
please call our Grand Haven office at 616-842-9210 or our
Coopersville office at 616-997-1786 and ask to speak to
a Case Coordinator. You may call either office and you will
be referred to the coordinator working the area in which
the client lives. |
